How the list is put together
Three layers stack up. Six items are always there - diapers, wipes, changing pad, cream, bags for dirty diapers and a spare outfit. The trip length adds the next layer, and the stage adds the last:
short outing = 6 base + stage items
full day = 6 base + 6 day items + stage items
overnight = 6 base + 6 day items + 6 overnight + stage items
Overnight includes the full-day layer rather than replacing it, so an infant packing for a night away gets 21 items against 15 for a day and 9 for a short outing.
How many diapers to count out
The list names diapers but not a number, because the number falls with age. Daily use is roughly:

The working rule is one per hour you expect to be out, plus two spares, rounded up for a long car journey. The item people most often skip is the second outfit - the first one goes on the baby after the first accident.

What is different about a newborn diaper bag?
Feeding and warmth. The newborn layer adds bottles and formula or a nursing cover, a swaddle blanket and spare pacifiers; the toddler layer carries snacks, a water cup, toys and a wipeable bib.
How long can made-up formula stay out of the fridge?
Guidance is two hours at room temperature, dropping to one hour once the baby has begun the feed. Prepared bottles keep about 24 hours in a fridge, and anything left in the bottle after a feed is discarded.
